Patient Forms

Patient outcome forms are useful tools to make measurements on your skin condition, such as its activity and the control you have over it. We also like to assess the impact the condition has on your quality of life as this can influence treatment decisions.

Even if you have previously filled out these forms, it is very useful to have an up to date form for each clinic visit so that your progress can be ascertained.

The forms have been validated and, in certain circumstances, are a requirement to get funding for some treatments.

They don’t take long to fill in and the results are kept confidential.

In order to keep confidentiality and to comply with GDPR, we ask that you don’t submit your name with the form. You will have been given a unique patient identifier code by Dr Marsland’s secretary, which consists of two letters followed by a number.

The forms that we currently use are:

Urticaria Activity Score (UAS7) -measures activity of chronic spontaneous urticaria over the last week

Urticaria Control Test (UCT) -measures, over the last month, control of all urticarial symptoms, including angioedema and inducible urticarias

Dermatology Life Quality Index (DLQI) – measures the impact of a skin condition on your life over the last week
